Personalised nutrition support helps manage gestational diabetes, resulting in a healthy pregnancy

CASE STUDY & SUCCESS STORIES

Name: Beena

Age: 32 years

BMI: 29

Location: Hyderabad, India

Case History: Beena is a young bureaucrat. She was 24 weeks pregnant and was diagnosed with gestational diabetes. Beena had a family history of diabetes, and her body was insulin-resistant. She was also diagnosed with gluten intolerance. Her blood reports showed a GTT of 135 mg/dl with vitamin D and iron deficiency. The diet that she was following showed a slow and steady increase in sugar, which left her tired and gave her a lot of sugar cravings. She wanted a nutrition program that managed her sugar levels, reduced her cravings, provided her with healthy alternatives and kept her energetic through the day. Hence, she approached us to help her in the management of gestational diabetes, ensuring a healthy pregnancy.

Approach by the best diabetic dietitian, Deepika Chalasani, Hyderabad, Telangana, India: We provided her with a personalised meal plan that supported her health goals and helped reduce insulin resistance in the body. Sufficient protein was given to her at the start of the day, along with a low glycemic load of food options that energised her. Soft fibrous food was given to her to ensure she could eat larger portions to provide complete nutritional requirements for the baby and her and ensure sugars were maintained. Micronutrients were given to help her with the optimum growth and development of the baby, along with meeting the requirements of the mother. Energy-dense and efficient complete protein foods like eggs, chicken, tofu, etc., were recommended to meet her calorie demands. An appropriate grain-to-vegetable ratio was given, which supported her gestational diabetes condition. Healthy and nutritious replacements for gluten were suggested, such as millets like jowar and, bajra and ragi. We recommended almond flour and coconut flour-based desserts, which did not increase her sugar levels. A variety of foods, such as seasonal fruits, vegetables and whole grains, were given to reduce her dependency on supplements. Casein-based dairy products like paneer hung curd and cheese were suggested to meet the calcium requirements as per NIN as she had diabetes. Deep sea fish like mackerel and salmon were recommended to help her maintain the omega-3 levels required for a healthy pregnancy. Her protein and micronutrient intake increased and gradually modified as she progressed in her goals and pregnancy. Appropriate, healthy, and safe lifestyle changes were made to support cravings, pleasure food, and social events step by step to support her pregnancy journey without compromising the health of the baby. Her gynaecologist approved the meal plans, and they were aligned with the doctor’s recommendation.

Results

Her fasting sugars were over 95 most of the time, which reduced between 85 – 90 and post-prandial to 110, thereby delaying the introduction of insulin. She was able to continue in the same manner for seven weeks. In the 32nd week, five units of insulin were added as she had gained weight due to excessive late-night social events and family rituals. The same continued till her delivery. She had a healthy baby with an average weight, and all her health parameters, including liver and sugar readings, were within range. She did not need to take any diabetic medications post the delivery.
