From Obesity to Optimal Health: Baldev’s 26 kg Weight Loss and Metabolic Recovery Journey by Deepika Chalasani, Best Nutritionist in Hyderabad, India

CASE STUDY & SUCCESS STORIES

Name: Baldev

Age:41 years

Location:Bachupally

Case Study: Baldev, a 41-year-old male, came to us with the primary goal of losing weight and improving his overall health. At the time of consultation, he weighed 113 kg with a BMI of 38 kg/m², placing him in the obese category. His body type was hypertrophic, and he had been diagnosed with diabetes, with an HbA1c of 7.2% and a TG/HDL ratio of 5.4, indicating poor metabolic health. His inflammatory profile was concerning, with an elevated hs-CRP of 18.3, showing ongoing systemic inflammation. Lipid investigations revealed raised LDL and total cholesterol levels, which required close monitoring. Additionally, he had deficiencies in both vitamin D and vitamin B12, further compromising his metabolic and immune functions. Urine tests indicated protein leakage, pointing towards early renal stress that required careful evaluation. As he wanted to have children, Baldev’s main objective was to bring his weight under control while simultaneously managing these comorbidities, including diabetes, dyslipidaemia, inflammation, micronutrient deficiencies, and renal risk, in order to reduce long-term cardiovascular and metabolic risks and improve his quality of life.

Baldev’s health journey called for a carefully designed plan that could address multiple concerns at once — obesity, diabetes, inflammation, dyslipidaemia, vitamin deficiencies, and early signs of renal stress. The nutrition approach was therefore not just about calorie reduction but about creating a structured, therapeutic meal pattern that worked with his body rather than against it.

His mornings began with two glasses of warm water, a ritual aimed at activating digestion, supporting hydration, and giving the metabolism a gentle push — a simple yet powerful first step for someone carrying excess weight and high inflammatory markers.

Breakfast was designed to be high in low-purine protein and low on the glycaemic load. Baldev was encouraged to start with a serving of local fruits such as papaya, muskmelon, pomegranate, and dragon fruit.Alongside this, steamed or sautéed vegetables were included to add fibre and antioxidants, helping reduce systemic inflammation. A protein-rich shake prepared with specialized formula powders became the anchor of the meal, providing clean protein for blood sugar stability, muscle preservation, and improved satiety.

For his mid-morning intake, a glass of diluted buttermilk blended with sabja seeds offered cooling hydration and gut-friendly probiotics. This light drink helped maintain digestive balance while ensuring he stayed nourished without burdening his metabolism between meals.

Lunch was structured to bring balance to the plate and discipline to portion control. Steamed or sautéed vegetables formed the bulk of the meal, accompanied by a moderate portion of brown rice and a simple vegetable curry. Protein is rotated between lean meats such as free-range chicken or freshwater fish, eggs, or homemade paneer with 2–3% fat, all in measured amounts to ensure adequate intake without excess fat.Probiotic support came from hung curd or Greek yoghurt, while a teaspoon of ghee was deliberately added to enhance absorption of fat-soluble vitamins, vital for correcting his vitamin D deficiency. By organizing the plate into half vegetables, controlled carbohydrates, and palm-sized protein, the meal created a steady energy profile and addressed both his diabetes and cholesterol concerns.

In the evening, cravings were handled strategically rather than suppressed. A small 30 g cube of date seed paired with 25 g of protein nachos from Fit4Life’s signature foods was given — a mindful snack that satisfied the taste buds without derailing the calorie framework. Dhal chaat or makhana was also given to add variety to snack time.

Dinner was kept deliberately lighter, as Baldev’s metabolic health required reducing the late-night load. The meal rotated between vegetable soups, steamed vegetables with Fit4Life’s green rotis, or dals paired with light curries. Plant-based proteins such as tofu were included to diversify his sources. Alternatives like bone soup, pesarattu with chutney, or bisibele bath with quinoa and black-eyed peas were recommended for variety, while root vegetables were excluded in the evening to avoid high-glycaemic spikes. Having dinner by 7:00 pm was emphasized as a key lifestyle anchor, giving his system enough time to process food before sleep and supporting better insulin sensitivity.

This plan was more than just a diet; it was a structured therapeutic intervention. Every element addressed a specific health marker: high-quality protein to stabilize blood sugar and reduce HbA1c, probiotic-rich foods for gut health and immunity, ghee and portion control for correcting micronutrient deficiencies, and anti-inflammatory vegetables to bring down hs-CRP. The controlled yet practical structure ensured that Baldev could follow it consistently, making it not just clinically effective but also sustainable for everyday life.

Result:

When Baldev first approached us, his foremost goal was to reduce weight and improve his health parameters to support fertility. At that time, he weighed 113 kg (BMI 38), was diabetic with an HbA1c of 7.2%, and had a high lipid profile with total cholesterol at 228 mg/dL and triglycerides at 180 mg/dL. His diet had shifted to vegetarian despite being a non-vegetarian by birth, leading to poor protein quality. He also had vitamin D and B12 deficiencies, protein leakage in urine indicating early renal stress, and no regular physical activity. Overall, his lifestyle and blood reports reflected high metabolic risk and low energy.

Through a targeted nutrition plan, Baldev gradually reintroduced suitable non-vegetarian proteins, corrected his micronutrient deficiencies with regular supplementation, and adopted a structured lifestyle that included daily physical activity. His meals were crafted to balance protein, fibre, and controlled carbohydrates while supporting gut health and reducing systemic inflammation.

The changes were significant. Baldev reduced his weight from 113 kg to 86 kg, losing over 27 kg and dropping two clothing sizes. His HbA1c improved from 7.2% to 5.8%, and his fasting blood sugar reduced from 133 to 103, moving him out of the diabetic range into near-normal control. His triglycerides reduced from 180 mg/dL to 144 mg/dL, reflecting a marked improvement in cardiovascular risk. His inflammation marker, hs-CRP, reduced from 18.3 to 9.65.

Beyond numbers, Baldev’s lifestyle saw a complete shift. He became physically active, felt more energetic, and carried himself with newfound confidence. His fertility parameters improved, and though his wife did conceive during this journey, the pregnancy unfortunately ended in abortion due to unrelated medical reasons. Even so, the couple now has a far healthier foundation to look ahead with optimism.

Today, Baldev is healthier, lighter, and more confident — a testament to how nutrition, supplementation, and lifestyle interventions, when followed consistently, can reverse metabolic disease and restore overall well-being. He is still continuing his journey of weight and fat loss, as he remains in the overweight category and wants to further improve his markers to ensure they are in the ideal range.
