CASE STUDY & SUCCESS STORIES

Name: Rama and Shree

Age:31 and 30 years

Location:Bangalore

Case History: Rama, a 31-year-old marketing manager, along with his wife Shree, approached us with health concerns that were beginning to impact his quality of life. With a family history of diabetes, Rama was already at a higher risk of metabolic disorders. His recent blood reports revealed elevated TSH and T4 levels, indicating thyroid imbalance. Additionally, his liver enzymes—SGOT and SGPT—were significantly raised, suggesting early signs of liver stress or a potential disorder.

He was also deficient in key micronutrients, namely vitamin D and B12, both of which are critical for energy, immunity, and neurological function. Rama reported symptoms like male pattern baldness—often associated with thyroid dysfunction and frequent sores that appeared after consuming sweets, pointing toward possible insulin resistance or glycation-related inflammation.

Given this complex interplay of genetic risk, hormonal imbalance, nutrient deficiencies, and symptomatic indicators, Rama was seeking a personalized nutrition plan to help him manage his condition effectively and adopt a healthier, more sustainable lifestyle.

Shree, a 30-year-old engineer with a history of fibroid surgery, presented with persistent shoulder, back, and neck pain. She further complained of skin issues, hair fall and imbalanced gut health. Laboratory investigations revealed a significant vitamin D deficiency along with low-normal iron and ferritin levels, likely contributing to her ongoing fatigue. While she reported some improvement in energy levels compared to previous months, she continued to experience noticeable exhaustion by evening that affected her daily activities.

With plans for conception in the near future, Rama and Shree were keen to optimize their physical health not only for her own wellbeing but also to create the best possible foundation for a future pregnancy. They sought to build strength and endurance through targeted nutritional support, aiming to correct their current deficiencies while enhancing her overall fitness in preparation for parenthood. As a working woman, Shree aspired to conceive naturally and experience a healthy, happy pregnancy while maintaining the stamina to manage her professional responsibilities comfortably. Her primary objectives included sustaining energy throughout the day, improving musculoskeletal health, and establishing dietary patterns that would support both her current lifestyle and future maternal needs

After evaluating Rama and Shree’s health conditions, symptoms, and dietary preferences, we designed a couple-centric nutrition strategy that is rooted in balance, simplicity, and sustainability aimed at restoring energy, supporting hormonal and liver health, and preparing for future parenthood.

To initiate their day on the right note, both were advised to begin with 1–2 glasses of warm water to promote hydration, stimulate digestion, and support natural detoxification. This simple ritual sets the tone for better metabolic activity and improved liver function especially crucial for Rama, who showed elevated liver enzymes, and Shree, who battles persistent fatigue and skin issues.

For breakfast, we introduced options that combined complex carbohydrates, plant-based protein, and healthy fats to ensure stable energy and hormonal support. Both were given a morning shake made with pea protein blended with nuts and water, alongside either quinoa upma with vegetables or sourdough toast with sautéed vegetables. These options provide sustained energy, reduce glycaemic load, and supply micronutrients critical for their respective needs—B12 and vitamin D in Rama’s case, and iron and vitamin D for Shree.

To improve insulin sensitivity, combat nutrient deficiencies, and support liver function, we integrated carbohydrate sources like hand-pounded rice, steel-cut oats, ragi, millet rotis, and quinoa in measured portions. These low glycaemic index options were carefully chosen to avoid post-meal sugar spikes and to reduce the burden on the liver, while their fibre content supported gut health, which was a concern for Shree.

Protein intake was personalized to suit their physiological needs. Rama’s plan included a higher volume of protein ranging from 40g of protein shake in the morning to tofu, paneer, and whole dals distributed across meals to support lean muscle preservation, metabolic regulation, and thyroid function. For Shree, who reported energy crashes and low iron, moderate amounts of easily digestible proteins like paneer, curd, tofu, and pulses were included consistently to rebuild strength and improve iron status without overwhelming her digestion. Greek yogurt and hung curd added probiotic benefits, supporting both gut integrity and hormonal balance.

To correct vitamin and mineral deficiencies, their meals were built around seasonal, colourful vegetables such as gourds, beetroot, carrots, broccoli, beans, and leafy greens, providing a spectrum of antioxidants and anti-inflammatory compounds. Fruits like watermelon, apple, pomegranate, and guava were preferred over high-sugar tropical fruits to protect liver health and avoid oxidative stress.

Mindful use of fats was emphasized through the inclusion of peanut oil for cooking and desi ghee in main meals. Ghee not only enhanced taste but also supported fat-soluble vitamin absorption and digestive strength. Fried or ultra-processed foods were restricted to no more than once a week to maintain hormonal and liver balance.

Snacks were kept simple and functional. Fresh fruits with coconut water, nuts and dates, prunes, figs helped Rama with his energy crashes and inflammation, while Shree benefited from this mid-day nourishment without adding metabolic strain. Evening tea or coffee was limited to moderate intake.

Dinner was deliberately kept lighter, focusing on steamed vegetables with fermented or millet-based foods such as ragi or lentil dosas, paired with tofu or sautéed vegetables. These meals supported overnight recovery, improved sleep, and enhanced musculoskeletal health—important for both individuals, especially Shree as she prepares her body for conception.

Given that the couple shares meals and routines, we provided practical, flexible options that could be adapted whether cooking together or separately. Proteins were encouraged to be prepped in advance to ensure consistency, and personalized variations were offered to align with taste preferences and nutritional goals. Through this collaborative, evidence-based dietary approach, Rama and Shree were able to make sustainable changes that not only addressed their current health concerns but also laid a solid foundation for long-term wellness, reproductive health, and resilience as a couple.
