Case History:Ankit was on the medications for non-alcoholic fatty liver disease. He was also moderately obese with BMI of 26. After analyzing his blood reports, we found that his indirect bilirubin was high i.e. 1.39 and other liver enzymes such as AST / SGPT, SGOT slightly over the range suggesting liver damage. Reports also showed destruction of red blood cells and low iron levels. He also had high bad cholesterol levels of 194mg/dl, high uric acid levels and high blood pressure. Apart from these conditions, he also faced frequent burps and flatulence. He was looking to heal his liver health along with correction of other blood parameters and maintain a sustainable healthy lifestyle.


Approach by the best dietitian in Hyderabad, Telangana, India: Upon analyzing his 24 hr dietary recall, we saw that he mostly consumed home cooked meals but his diet was lacking in good quality proteins and micronutrients rich foods. Minimal protein on somedays at breakfast and no protein on most daysat breakfast and inadequate protein at lunch was causing cravings towards the evening, which ultimately resulted in snacking and eating junk food / fast food towards later part of the evening and night. This late night eating habit caused tiredness and lethargy when he woke up in the next morningwhich stopped him from exercising in the morning and made him addicted to his early morning tea with sugar and subsequent no. of teas causing elevation in his liver parameters and other health parameters. Hence, we suggested him to start with 0.6-0.7g/kg body weight of protein and then increase gradually to 0.8. This was done by keeping a constant check on his creatinine levels as his uric acid levels was also high. Vitamin D and B12 level correction was done to help with overall health. We designed a meal plan with a macronutrient ratio of 50:15:35 with a focus on improving his liver health through nutrition. We suggested him good quality lean protein like homemade paneer made from low fat milk, moong dal, peas, hung curd, etc. which helped to increase satiety and promote fat burning. Complex carbohydrate, fiber rich foods like steel cut oats, ragi, millets, vegetables, quinoa were suggested to aid in metabolic flexibility and fat burn. Micronutrient rich fresh seasonal fruits like papaya, apple, kiwi, dragon fruit were suggested to take care of his sugar cravings. Healthy fats like ghee and cheese were suggested in very small portions to ensure the overall fat percentage did not go beyond 35 percent. Nuts such as almonds only was suggestedto ensure low fat nuts and satiety.Anjeer, dates were recommended in order to take care of his iron needs, sweet cravings and improve his washroom cycle.. We asked him to drink water every hour to avoid dehydration. As his snacking and nibbling in the evening and night had reduced, he had better quality sleep and was able to wake up fresh. Next morning. We suggested him to start walking and running to take care of his physical activity levels. Constant guidance and support was provided.



Results:

Within 3 months, his medications were completely off as his liver parameters were back to normal . His blood parameters became normal. His indirect bilirubin reduced to 0.82. His bad cholesterol levels reduced significantly to 61.5mg/dl. His energy levels increased. He no longer faced any kind of sweet cravings and got used to homemade foods so much that he no longer preferred eating outside. His uric acid levels reduced to 4 and he had lost 6 kgs. He also experienced belly fat reduction. Great inch loss was also seen.
