How Targeted Nutrition Helped a 47-Year-Old Fitness Enthusiast Improve Stamina, Gut Health, and Organ Function by Deepika Chalasani, Top Fitness Nutritionist in Hyderabad.

Case History:A 47-year-old finance professional with a strong passion for marathons, running, and high-intensity sports such as squash and weightlifting, approached us with concerns of persistent low stamina, gas, and bloating that had persisted for over three years. These issues had significantly affected his ability to participate fully in his preferred activities, including 10 km runs.


Despite having a healthy BMI of 24.1 kg/m², his lab reports revealed early-stage chronic kidney disease (stage 2) with a GFR of 85 mL/min, as well as signs of fatty liver. He was also experiencing elevated uric acid levels and high-normal urine specific gravity, suggesting compromised kidney function. Notably, his vitamin B12 levels were found to be excessively high, nearing toxicity.


He was on Evion LC and TendoCare supplements for right arm tendonitis. With these health challenges, he was actively seeking a nutrition plan to resolve his digestive discomfort, improve energy and stamina, and reduce his body fat percentage to 18%—ultimately allowing him to return to his athletic pursuits with renewed vigor.

As Akhil embarked on his nutritional journey, it was crucial for us to address his distinct medical conditions while creating a unified and sustainable dietary plan. This plan needed to support his energy and stamina levels. Listening to his dietary and lifestyle recall, we observed that his water intake was suboptimal and diet was a high protein diet that lacked diversity, with the same food consumed for both lunch and dinner.This needed to be replaced with an adequate and balanced diet, ensuring adequate protein split into 4 meals while ensuring that potassium, phosphorus, and sodium levels were kept within the lower limits of the Recommended Dietary Allowance (RDA). We ensured that he consumed adequate water of 3 liters throughout the day and not just during activity. We also ensured the inclusion of high-quality protein split into all 4 meals, providing no less than 0.8 grams per kilogram of body weight and later increased to 1- 1.2 grams per ideal body weight depending upon his exercise regime. To meet his daily protein requirements, vegetarian and animal protein sources such as protein shakes, whole dal salads, chicken, fish, and eggs were also incorporated.


To improve his gut health, we added fibre to his diet through salads, vegetables, and fruits low in potassium, such as papaya, dragon fruit, muskmelon, apple, berries, peach, and pineapple. Probiotics were included in the form of hemp milk, kefir, hung curd or yogurt. To cater to his specific dietary requirements, we incorporated complex carbohydrate sources like hand-pounded rice, quinoa, and millet. Food containing gluten, and lactose were restricted to improve the symptoms along with this generous serving of cooked vegetables prepared in innovative ways to meet his fibre and micronutrient needs, supporting his overall health goals. To prevent hunger pangs in the evening, we recommended healthy snack options like dal tikkis, pea protein shakes, a one-pot meal or tofu with sautéed vegetables, providing him with structured guidelines and meal plans for optimal food selection.


He was advised to start his day with 1-2 glasses of warm water and to stay hydrated by drinking a glass of water every hour, emphasizing water's role in boosting metabolism. For promoting health, we recommended cooking vegetables and curries in cold-pressed peanut oil, minimizing spices, avoiding deep-frying, and also maintaining 3 to 3.5 hours gap between meals, and ensuring at least 1.5 hours gap after meals and before exercise.

Results:

Akhil experienced a remarkable transformation in his energy levels and overall stamina after adopting the recommended diet plan. He was once again able to enjoy his favourite physical activities with renewed Vigorplaying squash three times a week, engaging in weight training thrice weekly, and completing a 10 km run every week. These improvements were accompanied by quicker recovery and sustained energy throughout his workouts.

His digestive health saw significant progress, with complete relief from bloating and post-meal gas. His bowel movements also became more regular and comfortable, reflecting a healthier gut.


Kidney function improved notably, as evidenced by an increase in GFR from 85 to 99 mL/min/1.73m²indicating enhanced filtration efficiency and renal health. Liver function and lipid markers also showed substantial improvement. His total cholesterol dropped from 214.5 mg/dL to 174 mg/dL, and LDL levels improved from 117 mg/dL to 106 mg/dL, reducing his cardiovascular risk.


A key metabolic markeruric aciddecreased from 8.24 mg/dL to 5.9 mg/dL, suggesting improved insulin sensitivity and a lower risk of conditions such as gout.

Akhil’s micronutrient levels also stabilized. His vitamin D levels rose from a deficient 21.19 ng/mL to a robust 64.1 ng/mL, supporting bone strength and immune function. Meanwhile, his previously elevated vitamin B12 levels dropped from 848 pg/mL to a healthier 490 pg/mL, reducing the risk of toxicity.


Overall, Akhil reported a dramatic boost in vitality. He achieved a 10 km run in just 53 minutes—a performance milestone he hadn’t reached in years.
