Case History:Shivesh ,a father andan IT professional, working in a desk bound profession consequently led him lead a sedentary lifestyle. He had been suffering from recurrent illness, which didn’t allow him to spend quality time with his children and family. He approached us as his lipid parameters were impaired mainly. Further his body composition had a disproportionate amount of fat to muscle ratio. He was also looking for a healthy weight gain through our program.By leading a healthy lifestyle, he hoped to set a positive example for his children.

Analysis by clinical nutritionist: He had a BMI of 20.7. His reports diagnosed that he was suffering from hypertrophic obesity, hyperlipidemia with high LDL of 163mg/dl, high triglyceridesof 301 mg/dl and low HDL of 23 mg/dlin blood and insulin resistancedue to high fat accumulation.Additionally, he had signs of fatty liverwith marginally low platelets count. His creatinine level was marginally high and uric acid level was also on the higher side. Further, hishs-CRP was 10.35 suggesting high inflammations in the body.

Approach by best clinical nutritionist in Hyderabad, Telangana:We looked at his food journal and discovered that his diet had an imbalanced macro nutrient composition. It was deficient in nutrients, particularly complex carbohydrates, healthy fats, whole grains, and enough protein. His body type was determined to be TOFI (thin on the outside, fat on the inside), thus our main goal was to improve his lipid markers. We designed a metabolic flexibility program for himto get his lipid and cholesterol profile in line.We gave him calorie and nutrient dense meals but lesser number of meals in a day.His meals consisted of whole grains like brown rice, good fat sources and protein like, green rotis, fish mutton, egg, whole dals and avariety of vegetables etc.To reduce his LDL, we gave him a whole grain: vegetable composition that was diverse. We gave him healthy fats in the form of omega 3 seeds, whole eggs, ghee but we ensured that the overall macronutrient percentage was carbohydrates: protein: fat percentage was 50:20:30. His lipid levels showed a sign of recovery, with a significant decrease in LDL (bad cholesterol). We then targeted on his weight gain by building more muscle. His daily caloric intake was gradually increased till it reached 2000kcal. He consumed foods with adequate proteins, and dietary fiber, such as hummus, greek yoghurt, tofu, super seeds, and homemadepaneer, among others. Additionally, we placed a high priority on enhancing his gut health by including omega 3 seeds, dates, and hung curd in his daily diet to enhance nutrient absorption, immunity, combat micronutrient deficiencies, and optimize overall health. We provided him with an exclusive meal plan to follow on the days he worked late-night office shifts with adjusted meal timings in consideration of his hybrid work style. This menu contained dishes that were suitable for the office, such as fish cutlets, tofu wraps, and sprouts chaat. We also helped him choose meals while he was travelling and on days when he wouldn't eat meat. Additionally, we provided him with advice on how to create an effective fitness regimen to support and enhance his health.

Results:He saw a significant improvement in his lipid profile after making dietary modifications, with a drop in LDL from 163 mg/dl to 109 mg/dl, an increase in HDL from 23 mg/dl to 32 mg/dl, and a drop in triglycerides from 301 mg/gl to 199 mg/dl over the course of 70 days. His HbA1C level decreased, indicating progress towards insulin sensitivity. He also had an increase in platelet counts, a decrease in hs-CRP, and an improvement in liver markers, all of which point to improved liver health. His levels of creatinine and uric acid were also within the acceptable range. He gained 2 kgs in weight in addition. He feels healthy, energetic and his gut health has improved. He has inculcated the improvisations suggested by us and is determined to continually practice it to optimize his overall health. He is thrilled to have created this way of life for himself and is able to encourage and drive his kids to do the same.
