CASE STUDY & SUCCESS STORIES

Name: Naila Nazneen

Age: 31 years

Gender: Female

BMI: 35 kg/m2

Case History: Naila approached us with irregular menstrual cycles which were diagnosed as PCOD and resultant weight gain. Her energy levels were low and she wanted to lose weight. She used to feel very lethargic and used to get tired easily. She registered for a weight loss & PCOD program with one of the known health firms but could not continue for more than a couple of weeks as the calorie deficit and portion-controlled diet were making her energy levels lower. At times she would also feel dizzy and was not able to do even her household chores.

Analysis by best female nutritionist: Past history, dietary recall and lifestyle choices. She was also pre-diabetic. Her hsCRP score was high which indicated that she had inflammation in her body. She had higher than normal LDL levels and had low iron and folate levels.

Approach by best clinical nutritionist/dietician: We analyzed her food diary and saw that her diet was lacking fibre and protein and her food intake was lacking vital nutrients that were causing vitamin and mineral deficiency and inflammation. We started by including fruits, vegetables and adequate protein in her meals especially breakfast so that she could start her day with a boost of energy. Earlier she was having dates after waking up to increase her energy levels in the morning which we moved to the evening as dates are natural sugar which the body needs but not great to start the day with. Dates in the morning can give instant energy but also the energy does not sustain for long. As she is a non-vegetarian by birth, we looked to introduce some traditional food into her diet, which she had eliminated as she thought it was not good for her. For e.g. Mutton liver was given twice a week to keep her folate levels in check, seeds were also included in her diet to include essential fatty acids in her diet and support metabolic flexibility. Her meal timings were not proper so we set her meal timings so that she could manage it with her exercise regimen. She used to drink less water and we educated her on importance of water and monitored her water intake on daily basis. As a part of the blood report analysis, we also observed that she was slowly inching towards an insulin resistance condition as she had a PCOS history. Therefore, we ensured that we gave her a meal plan that proactively reversed these conditions. She started following the meal plans and liked all the recipes and felt excited about following the meal plan. Since she had a history of PCOS, she was very clear that she did not want to gain weight so we paid extra attention to her physical activity levels so that she could continue for the rest of her life. We included black coffee before her workout sessions as a part of her routine in order to shift her body into fat-burning mode. While she was happy with the progress, she was concerned about sustaining the weight loss as there were impending weddings in the family and subsequently Ramzan and Eid would also be there which could cause a lot of weight gain as she had in the past. We gave her meal plans, guidelines to keep the weight gain to a minimum and also reverse the weight gain after the celebrations and events so that she could sustain her weight loss.

Results: After making lifestyle changes, she could observe that she lost 7 kgs in 3 months, her periods cycle got back to a 35-day cycle without any requirement of medication. Her overall energy levels improved. Her periods became normal without any medication. Despite attending functions, she was able to sustain weight loss for a year and her period cycle continues to be in a 35-day cycle.
