Case Study: Rohan, a 35-year-old businessman, is a known case of hypertension and hypothyroidism for which he has been on regular medication. On clinical assessment, his body mass index was found to be 26.87 kg/m², placing him in the obese range with a hypertrophic body type. His lipid profile revealed a TG/HDL ratio of 2.1 and elevated levels of LDL and total cholesterol, both of which require close monitoring. Rohan’s health journey called for a carefully designed plan that could address multiple concerns simultaneously obesity, hypertension, thyroid imbalance, and most critically, his elevated LDL and total cholesterol. The nutrition approach was therefore not just about cutting calories, but about rebalancing fats, optimizing fibre intake, and introducing therapeutic foods that actively support lipid lowering and cardiovascular health.

n, and prepare his system for better nutrient absorption. Breakfast was deliberately crafted to be protein-rich yet heart-friendly. A protein shake blended with nuts, seeds, and vegetables provided satiety, muscle support, and steady energy release while avoiding refined carbohydrates that could worsen cholesterol and weight. Options like idli, cheela, or poha were paired with vegetables to enhance fibre intake, a key factor in lowering LDL cholesterol.


Mid-morning hydration came through light but purposeful inclusions like buttermilk or hung curd, which added probiotics for gut health while also being low in fat and cholesterol. This not only supported digestive balance but also helped in improving metabolism, especially important with his thyroid condition.


Lunch was structured to maximize fibre and plant protein while controlling hidden fats. A base of fresh salads and leafy vegetables was combined with legumes, chana, or dals for soluble fibre proven to reduce LDL cholesterol. Homemade paneer, tofu, or moderate curd acted as protein anchors, while the controlled addition of whole grains like jowar roti or brown rice ensured slow glucose release and steady energy. Ghee, used sparingly, was included strategically for nutrient absorption but strictly portion-controlled to avoid cholesterol overload.


The evening meal plan was aligned to handle cravings without resorting to fried or processed snacks. Fresh fruits with curd or a glass of buttermilk were encouraged, both low in saturated fats yet refreshing and satisfying. These choices helped keep calorie intake in check while also preventing spikes in hunger that could lead to poor food decisions.


Dinner is a family meal at his home, and the family’s usual practice includes snacky items such as pav bhaji, pasta, or pizza, etc. To ensure that he enjoys the family meals and also stays focused on his health goals, we gave him a protein-rich breakfast, a wholesome lunch, and a sumptuous snack so that he did not feel very hungry for dinner. At dinner, we asked him to start his meal with a broth-based vegetable or soup, or a portion of less-cooked vegetables in the form of steamed tadka vegetables, stir-fry greens with tofu, or home-made paneer with 3% fat, followed by the carbs made for the family meal.


This structured approach went beyond a typical diet it was a therapeutic intervention targeted at lowering cholesterol. Every component played a role: high-fibre legumes and vegetables to bind cholesterol, lean proteins to improve metabolism, probiotic support to balance gut health, and portion-controlled good fats to aid nutrient absorption without raising LDL. By integrating these strategies consistently, Rohan’s plan created a path not only to reduce his LDL and total cholesterol but also to improve his overall cardiovascular resilience, making it both sustainable and clinically effective.

With consistent adherence to the structured nutrition and lifestyle plan, Rohan showed remarkable improvements in his metabolic and lipid profile over a short span of time. His thyroid function showed positive movement, with TSH reducing from 6.58 to 5.513, indicating better thyroid regulation without any medication.


Most importantly, his cholesterol management goals were successfully achieved. His total cholesterol dropped from 209 to 155 mg/dL, bringing it well within the desirable range. His LDL cholesterol reduced from 137 to 99 mg/dL, a significant improvement that lowered his cardiovascular risk. Triglycerides, too showed encouraging progress, reducing from 109 to 85 mg/dL, reflecting improved fat metabolism.


Weight reduction was gradual yet sustainable, with his weight coming down from 90 kg to 87.2 kg, and a 2.5-inch reduction on his waistline, reflecting fat loss while maintaining lean body mass.
