Case History

Divya, a homemaker and a mother of a young child living in a joint family, was feeling lethargic and tired throughout the day. She felt out of energy and had lack of motivation and strength. She was suffering from PCOS and she used to complain for migraine and constipation quite often. She was also overweight which made her health condition more difficult to manage. She used to face joint pain and palpitations. Her triglycerides levels were borderline of 142mg/dl. She has tried many diet programs in the past but none of them gave holistic lifestyle change programs that supports all her conditions. She was not able to lose beyond a couple of kgs even though she followed the program for several months. Some of them had quick diet fads such as 50 ml of apple cider vinegar before every meal, coconut oil shots etc. She is keen to have another child and therefore was looking for a program that supports her overall health and well-being.


Analysis by the best nutritionist: Past history, dietary recall and lifestyle choices. Her blood reports suggested severe protein malnutrition as her albumin levels were 3.6 and her Hs-CRP levels detected inflammation. Her Hb levels was 11.9 and all her anemia related parameters such as iron, ferritin, MCV, MCH, MCHC, PCV were low normal or below the range.

Upon analyzing her 24 hr dietary recall, we found that there were lots of gaps between the main meals as she was starving, which led to severely low protein consumption throughout the day. The foods which she was eating were very rich in simple carbohydrates which was causing fluctuation in her insulin and sugar levels. Her meals lacked in fiber, protein and various micronutrients like iron, b12, calcium and vit D. Hence, we suggested her to start with a meal plan with macronutrient ratio of 48:17:35. We included vegetable protein shake, superseeds, protein powder to meet the nutritional deficiencies. We also provided her with organ meat and eggs to meet her protein and iron needs. We asked her to prepare her protein sources in bulk and store it in freezer to save time and maintain consistency in protein consumption. A gap of 3.5-4 hour between each meal was suggested and we gave her low glycemic food that were locally available like millets, , berries, brown rice, papaya, oranges, , and other citrus fruits in order to maintain her sugar and insulin levels. A variety in the diet and meals were given in the form of simple to make recipes. We also ensured that food that was being consumed in her home were included.in her meal plan so that she did not feel left out while having family mealswhich helped her to remain consistent in the nutrition journey with us. The nutritionist ensured that she is consistent with the consumption of approx. 400-500g of vegetables every day in order to reduce her triglyceride levels. To reduce inflammation in the body, we provided her with vegetables, whole grains and soups. Liver, dates and seeds paste were incorporated to improve her iron and hemoglobin levels. Constant support and guidance was provided and regular monitoring was done.



Results

With our nutrition support, diet and guidance, her Hs crp levels reduced to 2.56 which means the inflammation in the body reduced. She was able to make conscious, healthy and sustainable food choices even while traveling and during festivals and occasions. Her triglyceride levels reduced to 92mg/dl. She lost 6kgs in 5 months. Her nutrient deficiencies came to normal range. Due to increase in fiber intake, her episodes of constipation reduced. Her washroom cycles improved and her periods became regular. Her recent scan did not have any trace of PCOS. She no longer felt lethargic and was able to perform her daily routine with ease, more energy and without any discomfort. All her anemia parameters showed marked improvement. There was great improvement in her overall lifestyle behavior which is helping her manage her health issues and weight.
